Who Can Connect With us?
Mothers are typically connected to our service through referrals from their healthcare providers or any providers involved in pregnancy, birth, and postpartum care.
Including but not limited to, obstetricians, Gynecologists, midwives, family physicians, hospital /walk in clinic maternity units, pelvic floor physiotherapists, lactation consultants, and public health nurses etc.
Postpartum 411 Care Navigation is not a medical or emergency service. We do not provide clinical diagnosis or treatment. Instead, we act as a dedicated support layer by helping individuals access the right care, at the right time.
Postpartum 411 Connect will contact the patient within 48 hours, of the referral or the patient may contact Postpartum 411 Connect directly. Whichever occurs first. A birth mother may not refer herself as we do not provide medical expertise. We are a Postpartum Care navigating Agency.
